General comments

vancomycine dosering: oplaaddosis 20 mg/kg, vervolgens 35 mg/kg continu per 24 uur, OF oplaaddosis 30 mg/kg, vervolgens 17,5 mg/kg 2 dd. Onderhoudsdosering vancomycine verder afhankelijk van nierfunctie en van gemeten spiegels, zie bij vancomycine en bij TDM vancomycine

  • dosering flucloxacilline iv zonodig 1000 mg 6dd
  • alternatief bij beta-lactamallergie (bij gebleken gevoeligheid):
    • clindamycine iv 600 3ddv, tenminste 2 weken
  • bij een niet IgG-gemedieerde penicillineallergie (bv. rash) Cefuroxim 1500mg 3dd
