followed by

clindamycine po 600mg 3dd 2 to 4 weeks

benzylpenicilline iv 1million. IU 6dd 4 to 6 weeks

or

benzylpenicilline iv 6million. IU continuous per 24 hours 4 to 6 weeks
Medication:

cefazoline iv 1g 3dd 2 weeks

followed by

clindamycine po 600mg 3dd 2 to 4 weeks

Bij niet-IgE gemedieerde penicilline-allergie

General comments

Afhankelijk van kliniek en gekweekte verwekker kan na 1-2 weken overgestapt worden op orale behandeling.

  • Benzylpenicilline bij voorkeur continu doseren (6 milj IU per 24 uur).
  • Bij morbide obesitas met een lichaamsgewicht vanaf 180 kg dient de clindamycine dosering te worden verhoogd van 600 mg naar 900 mg per keer.
