Alternatief bij niet-IgE gemedieerde penicilline-allergie 

General comments

Evt. switch naar oraal na 2 weken intraveneuze therapie aan de hand van de kweekuitslagen

  • Antibiotica aanpassen/stroomlijnen op basis van de kweekuitslagen.
  • Flucloxacilline bij voorkeur continu doseren ( 6000 mg/24 uur).
  • Aminoglycoside: gentamicine 1dd 5 mg/kg. Bij een BMI > 27 dient het doseergewicht te worden berekend met de formule: Doseergewicht = IBW + 0.4 * (TBW – IBW) .
  • Beoordeel gerapporteerde penicilline allergie aan de hand van de anamnese.
